When you make the decision to pursue hospice care, the hospice physician reviews your medical record, assesses your medical diagnosis, and helps make the determination that the diagnosis is terminal.

Social Services

The end of life journey can be a difficult time with many unknowns. Every patient under our hospice care is assigned a social worker whose job it is to help our patients and their loved ones navigate these challenging areas.


Every patient in our hospice care is assigned a nurse case manager. Hospice nurses work to coordinate all aspects of care for the patient, their caregivers, and their loved ones.

Learn how important bereavement is in the dying process. Agape is equipped with a bereavement team ready to help you with grief and the mourning of a lost loved one.

Hospice Aide

Our hospice aide service is an essential element to our holistic approach to end-of-life care. Every patient, assuming the accept our offering of this service, is assigned a hospice aide who comes out a minimum of twice a week to assist with showering, bed baths, dressing, and general hygiene care.
